So, when cards say “prevent all effects of attacks done to this Pokemon (damage is not an effect)” — would the damage-counter-placing that is so common on ghost types be prevented? Does the placing of damage counters not count as damage? Is it an effect?

Yep, direct placement of damage counters is considered an effect and not damage. Hence why placement of damage counters also isn’t affected by weakness or resistance. Some early cards were inconsistent about the difference though, such as the first print of Rainbow Energy.
